Advance Directive Completion
Trident Senior Health & Heartland Hospice have partnered to provide you or your family with assistance on advance directives completion, which includes Healthcare Power of Attorney and the living wills. They will now be able to be offered in the Trident Senior Health office. Most times can be done same day in conjunction with your office visit or can be arranged to be done at your home or family member’s home. Whichever is most convenient for the you and your family.
